ACCEPTANCE: I acknowledge receipt of the Facility Access Cards identified above. I further acknowledge the waiver as set forth below and agree to its terms. I have also been provided with, read, and agree to abide by the Rivers Edge CDD Polices Regarding District Amenity Facilities. I als o understand that I am financially responsible for any damages caused by me, my family members or my guests and for any damages resulting from the loss or theft of my Facility Access Card. I further understand that I am financially responsible for any damages caused by any person(s) whom I a llow to reside in my home by any formal or informal agreement and with whom I share my rights to access the amenity center facilities by granting said person(s) use of my Facility Access Cards. I also agree to report to the Amenity Office any individuals resident in my home, whether related to me or not, in order that the Amenity Center is able to keep track of the number of non resident guests I bring to the amenity center facilities. The undersigned agrees and acknowledges that the above information is true and correct. It is understood that User and Guest cards are the property of the Rivers Edge Community Development District (RE-CDD) and are non-transferable except in accordance with the District’s rules, policies and/or regulations. In Consideration for the admittance of the above listed persons and their guests into the RiverTown facility owned and operated by the RECDD, the undersigned agrees to hold harmless and release the Rivers Edge CDD, Government Management Services, its agents, officers and employees along with Vesta Property Services, its agents, officers and employees from any and all liability for any injuries that might occur in conjunction with usage of RiverTown ( including but not limited to: fitness rooms and equipment, swimming pools, tennis courts, volleyball courts, playground equipment, club house space). Nothing herein shall be considered as a waiver of the Districts sovereign immunity or limits of liability beyond any statutory limited waiver of immunity or limits of liability which may have been adopted by the Florida Legislature in Section 768.28 Florida Statutes or other statute.

ACCEPTANCE: This Authorization and Release (“Release”) is by and between the undersigned individual (“You”) and Rivers Edge Community Development District and its affiliates, related entities, directors, officers, shareholders, representatives, successors, assigns, licensees, employees, and any person, corporation or entity acting under its permission or with its authority, or for whom it might be acting, including anyone distributing or disseminating communications by or regarding Rivers Edge CDD (the “Licensed Parties”). For good and other valuable consideration, the receipt and sufficiency of which are hereby acknowledged, You hereby irrevocably give and grant to Rivers Edge CDD perpetually, exclusively, and for all media throughout the world (including print, non‐theatrical, home video, CD‐ROM, internet, and any other electronic medium presently in existence or invented in the future), the right, permission and license to incorporate, create, produce, publish, reproduce, distribute, and/or otherwise use (alone or together with other materials), in whole or in part, Your name, photographic image, video, likeness or sound recording taken of You as a result of Your participation in the activities of Rivers Edge CDD (the “Activities”). You hereby waive all rights of inspection or approval with regard to any recording, taping, reproduction, proposed printed, audio or video publication, and/or other use of Your name. You also hereby waive, release, discharge and agree to hold harmless Rivers Edge CDD and the Licensed Parties from and against any and all liability to You or any third parties resulting from their use of Your name or your participation in the Activities. You agree that Your participation herewith is voluntary and You assume complete responsibility for Your actions in connection herewith. Nothing herein shall constitute any obligation on the part of Rivers Edge CDD or the Licensed Parties to make any use of any of the rights granted herein or to prepare, produce, exhibit, distribute or exploit the property. Rivers Edge CDD shall have the right to assign its rights hereunder, without your consent, in whole or in part, to any person, film or corporation. You hereby agree that You will not bring, or consent to others bringing, any claim or action against Rivers Edge CDD and the Licensed Parties on the grounds that anything contained in the Activities, property, or in the advertising and publicity used in connection herewith, is defamatory, reflects adversely on you, violates any other rights whatsoever, including, without limitations, rights of privacy and publicity. You hereby release Rivers Edge CDD and the Licensed Parties from and against any and all claims, demands, causes, and actions, suits, costs, expenses, liabilities, and damages whatsoever that you may hereafter have against Rivers Edge CDD. This Release shall be binding upon my heirs, successors, assigns, and personal representatives, without regard to whether it is expressly acknowledged in any instrument of succession or assignment. You affirm you are eighteen (18) years of age or older and/or are parent or guardian of those in your household under eighteen (18) years of age.

ACCEPTANCE: I, the undersigned, on behalf of myself, my personal representatives, my minor children and my heirs hereby voluntarily agree to indemnify, defend, release, hold harmless, and forever discharge the Rivers Edge Community Development District (the “District”), Governmental Management Services, LLC, Vesta Property Services, Inc., and their respective present, former, and future supervisors, staff, officers, employees, representatives, agents, and contractors (together, the “Releasees”) from any and all liability, claims, lawsuits, actions, suits, or demands, whether known or unknown, in law or equity, for any and all loss, injury, damage, theft, real or personal property damage, expenses (including attorney’s fees, expert witness fees, paralegal fees, costs and other expenses for investigation and defense and in connection with, among other proceedings, alternative dispute resolution, trial court, and appellate proceedings), and harm of any kind or nature arising out of, or in connection with, my, my children’s and my guests’ participation in the special event(s) hosted by the District(s) and any other use of the Districts’ property (together, the “Activities”). I acknowledge that I have freely chosen to participate in the Activities, and I agree that by participating in the Activities, I will fully comply with all rules, policies, or other requirements, or I will be required to leave. I also acknowledge that certain dangers are inherent in participation in certain recreational activities, including but not limited to bounce houses, sports competitions, and other physical activities, including risk of physical injury or death. I expressly acknowledge that I assume all risk for any and all injuries and illness that may result from my, my children’s and my guests’ participation in any and all of the Activities. This Waiver and Release is binding upon me, my children, my guests, my heirs, executors, legal representatives, and successors. The provisions of this Waiver and Release will continue in full force and effect even after the conclusion of my use of the District’s property. The provisions of this waiver of liability may be waived, altered or amended or repealed, in whole or in part, only upon the prior written consent of the District. I understand that this document is intended to be as broad and inclusive as permitted by the laws of the State of Florida. I further understand that nothing in this waiver and release shall constitute or be construed as a waiver of the District’s limitations on liability contained in Section 768.28, Florida Statutes or other statute or law. I agree that if any portion of this waiver and release is deemed invalid, that the remainder will remain in full force and effect. I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT AND FURTHER UNDERSTAND THAT BY SIGNING THIS DOCUMENT I AM WAIVING CERTAIN LEGAL RIGHTS AND REMEDIES. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL. I UNDERSTAND THAT BY SIGNING BELOW, SUCH WAIVER AND RELEASE, INCLUDING ALL OF THE TERMS IN THE PRECEDING PARAGRAPHS, SHALL APPLY EACH AND EVERY TIME I, MY CHILD, OR MY GUEST UTILIZE THE DISTRICT’S FACILITIES OR LANDS.

The undersigned, a home owner within the Rivers Edge CDD, agrees and acknowledges that it will not provide Facility Access Cards to any Tenant (as used herein, the term Tenant shall include all family members of the specifically named Tenant) without first providing a RiverTown Facility Registration Form (the “Registration”), executed by Tenant, to a RiverTown staff member. Should the undersigned provide Facility Access Cards to a Tenant without providing a RiverTown staff member with a Registration signed by Tenant, the undersigned agrees to be financially responsible for any damages caused by Tenant and agrees to indemnify the Rivers Edge CDD, Government Management Services, its agents and Vesta Property Services from any and all liability for any injuries that Tenant may sustain in conjunction with the usage of the RiverTown facilities. Nothing herein shall be considered as a waiver of the Districts sovereign immunity or limits of liability beyond any statutory limited waiver of immunity or limits of liability which may have been adopted by the Florida Legislature in Section 768.28 Florida Statutes or other statute.
